Before reopening, all workplaces are required to perform the following, even if you were previously open on the “Essential Services” List:
Fill out a COVID19 Control Plan. This plan must be posted in your workplace: English * Espanol
Fill out a self-certification and post at the workplace: English * Espanol
The City of Medford is requiring that you complete the self-certification form and send it to the Board of Health via this link: https://app.smartsheet.com/b/form/02b7411955b14e6eac224ce385000cb0. This must be completed and returned to the Board of Health prior to your reopening.
Train employees on COVID prevention. Training must be effective for employees to understand and implement the COVID safety measures put in place.

The Medford Health Department will be conducting investigations that come in as complaints; it is the responsibility of the business owner to maintain compliance.
