Medford to Expand Social Care Referral Program with Partnership with Unite Us

The City of Medford is partnering with Unite Us, a digital referral platform, to participate in the Unite Us Massachusetts network, Mayor Breanna Lungo-Koehn announced. Through Unite Us Massachusetts, the City will be able to securely and electronically direct people to targeted care options and track their progress. 

Unite Us is a shared technology platform that works with community partners to help partners provide social care, including sending and receiving electronic care referrals, addressing the community’s social needs, improving health, and identifying service gaps. Unite Us Massachusetts is a coordinated network of health and social service providers that are connected through the Unite Us platform.  

“Our external partner agencies do amazing work delivering care to those in need, but we could streamline our workflow efficiency by better tracking the progress of our referrals and ensuring that the needs of our community are being met,” Mayor Lungo-Koehn said. “Unite Us will provide the community and our partners with consistency and accountability when it comes to social care.” 

Medford will utilize this platform to connect partners that provide essential health and social service care to the community. Partners of the City will be able to use the platform for free, eliminating the financial burden for care providers. City employees and community partners will be able to refer people in need to the correct care providers to efficiently connect people to help. 

“The ease of access to health and social service providers that Unite Us will grant the City will transform the way people are able to get care here,” said Penelope Funaiole, Prevention and Outreach Manager. “Our goal in the Office of Prevention and Outreach is to promote mental and physical wellbeing by making resources easy to access for multilateral care.”
